If any of all of the requested information marked ‘required’ is not provided, Melbourne Fringe Ltd will not be able to provide the services to you and membership may not be granted. Melbourne Fringe Ltd will provide you with access to and the ability to correct your personal information held by us on request by you to the extent permitted or required by law.

I consent and authorise Melbourne Fringe Ltd to use and disclose all of the information on this application form and other information that I provide to Melbourne Fringe Ltd and any other persons (including without limitation a related body corporate, agent or contractor) for the purpose of providing services, including by means of research, marketing, product development and providing services to members in connection with the administration of the program.
